Statewide Service Coordination

At PAHM, our focus is on increasing positive health outcomes and the quality of life of individuals in our community. We achieve this by spending time with each person one-on-one, building a supportive team, and working to understand the details of each unique case. We help individuals reach medical and social goals and prevent issues and risks through identifying and addressing their root causes. PAHM is qualified through the Office of Long-Term Living (OLTL), Department of Human Services (DHS), to provide Service and Supports Coordination for eligible individuals throughout the entire state of Pennsylvania. 

Services Offered

PAHM provides Service Coordination through the Managed care organizations (MCOs) chosen by the Department of Human Services to deliver Managed Long-Term Services and Supports (MLTSS). Supports available to individuals are specialized for a wide array of health and social needs, including and not limited to physical disabilities, mental disabilities, brain injuries, cognitive impairments, mental health concerns, individuals who are aging, and more.

Who is eligible?

Pennsylvanians who meet the level of care needs for a Skilled Nursing Facility that are between 18 and 59 years of age. Be capable of directing your own care and have a medically determinable physical impairment. Be determined financially ineligible for the Medicaid (MA) program.

OBRA Waiver

Who is eligible?

Pennsylvanians between the ages of 18 and 59 who have a severe developmental physical disability requiring an Intermediate Care Facility/Other Related Conditions (ICF/ORC) level of care. The disability must result in substantial functional limitations in three or more of the following: self-care, communication, learning, mobility, self-direction and capacity for independent living. You must also meet the financial requirements as determined by your local County Assistance Office.

Community HealthChoices (CHC) Waiver

Who is eligible?

Pennsylvanians and individuals who are 21 or older, and have both Medicare and Medicaid, or receive long-term supports through Medicaid who need help with everyday personal tasks.

  • Home Health Care including Nursing, Aide and Therpay Services
  • Non-Emergency Medical Transportation
