Supports Program (SP) Waiver
The Supports Program is a Medicaid home and community based services (HCBS) waiver program. It provides services for New Jersey Adults with intellectual and developmental disabilities who live in their family home or their own apartment or home, to assist them to live as independently as possible.
Community Care Program (CCP) Waiver
The Community Care Program is a Medicaid home and community based services (HCBS) waiver program. It provides services for eligible individuals who live in a provider-managed setting, such as a group home or supervised apartment, or who live in their family home or in their own apartment or home, to assist them to live as independently as possible.

We believe in taking a person-centered approach to the care that we deliver. We work closely with individuals, families, and collaborative care teams to develop care plans that fit the needs of everyone we serve - taking into account that each person we assist has their own talents, skills, interests, and needs that are unique to them.
We train our Direct Support Professionals (DSPs) on the Individual Support Plan (ISP) that is written and maintained by the Supports Coordinator (SC). Furthermore, our team of Program Specialists and Directors ensure that's the DSP understands the role they play in the person's Everyday Life.
